So what next?? How to solve.. the issue is been there since very long time

No not the single big black dots.. the smaller tiny dots

Hi I'm getting a lot of grainy, black dots which are static.. I've changed and tried all different combinations of settings and waited for updates and reinstalled the game but still couldn't solve it.. it's just too grainy..kindly help me I5 6500, GTX 1060 6GB, 12 GB RAM.
